Notice of Electronic Disclosure of Health Information
In accordance with the Texas Medical Records Privacy Act (HB 300, Tex. Health & Safety Code Ch. 181), Oak & Willow Counseling may disclose your protected health information (PHI) electronically only for treatment, payment, or health care operations, or as authorized in writing or required by law. Any other electronic disclosure of your PHI requires your separate written authorization.
How we may use and disclose your information
- Treatment — to provide and coordinate your care.
- Payment — to bill and receive payment for services.
- Health care operations — to support the quality and operation of the practice.
- As required by law — when disclosure is mandated by applicable law.
Any other electronic disclosure of your protected health information requires your separate written authorization, which you may revoke at any time.
Your rights
- To request access to your records (generally within 15 business days).
- To request amendments to your information.
- To request restrictions on certain uses and disclosures.
- To receive an accounting of disclosures.
- To be notified in the event of a breach affecting your information.
